The event was held at the John F. Kennedy Memorial Center for the Performing Arts. The honorary chairpersons were first ladies Mrs. Gerald Ford, Mrs. Lyndon B. Johnson, Mrs. Richard M. Nixon and Mrs. Harry S. Truman. Gold presidential seal on front cover. Added bonus includes the original envelope but the envelope is in tattered condition.
